Our specialized, comprehensive rehabilitation program thoroughly prepares our residents and their caregivers for a successful – and rapid – return home. It is covered by Medicare – and it works!
 
The eight-point program includes:
  • A professional team exclusively devoted to providing physical, occupational and speech therapy to seniors.
  • Flexible, personalized scheduling which allows therapists to work with residents more often, for shorter, more effective periods of time.
  • A comprehensive exercise program precisely tailored to each resident’s specific needs.
  • A multi-disciplinary home assessment at the outset of the program to ensure that therapy programs are customized for a successful return home.
  • A simulated home environment where residents practice activities of daily living to instill confidence in their caregivers as well as themselves.
  • Caregiver and family training and support.
  • A personalized Return Home Packet with a detailed home exercise program, list of equipment recommendations and other suggestions designed to ensure a successful and enjoyable return home for both residents and caregivers.
  • Ongoing communication with physicians to provide information about progress and the level of independence achieved by their residents, as well as therapists’ recommendations for returning home.
